By signing the Membership Commitment Form, you are committing to donate $100 each quarter to the charity chosen by the membership.  If you are under 35 years of age, the commitment is $35 quarterly. This is not a legal commitment; it is an honor commitment. 

At the first meeting you attend, we ask that you complete and sign the Membership Commitment Form, a copy of which is attached below for you to review.

"I understand that I am making a commitment to Treasure Lake / DuBois - 100 Women Who Care to make an annual donation of $400 ($100 donation at each of four quarterly meetings). If I am 35 or under, I am making a commitment to an annual donation of $140 ($35 donation at each of four quarterly meetings). These donations will be made directly to local 501(c)(3) charities in the Treasure Lake, DuBois and surrounding area. I understand that even if I did not vote for the charity chosen by majority vote, I will fulfill my donation commitment. I also understand that if I am not able to attend a quarterly meeting, I will provide my check(s) to another member to deliver to the meeting, or mail to Treasure Lake / DuBois - Women Who Care after the meeting (notification will be sent via email). I understand that I must be present to vote. My commitment will automatically renew, for successive one year periods, unless notice is given. I can resign at any time. Agreeing to this is an honor pledge, not a legal agreement."
